Please print the New Student Health Services Checklist and use it to ensure you have completed your Health Services requirements prior to arrival at ºÚÁϳԹÏÍø±¬ÍøÕ¾.

• All ºÚÁϳԹÏÍø±¬ÍøÕ¾ students must comply with  as well as requirements put in place by the College. Maine allows medical exemptions ONLY () . In the event of a disease outbreak, students who are not immunized may be asked to leave campus for an extended period.
• ºÚÁϳԹÏÍø±¬ÍøÕ¾ requires the following vaccinations: ºÚÁϳԹÏÍø±¬ÍøÕ¾ College Immunization Requirements
• Obtain a thorough immunization record. Many students can access their records through their health care portal, school or state registry.  Do not wait until you have an appointment to do this.
• Please note that some vaccines have minimum age and interval requirements.  If you do not meet those minimums, your vaccine must be repeated.
• Review your immunization record for required immunizations. Plan to receive any missing immunizations. If all immunizations will not be complete by June 20^th you must complete the Request for Provisional Admittance  and upload both the request and your incomplete immunization records to the Student Health Portal by June 20th.
• Upload documentation of immunizations received over the summer as soon as possible.

ºÚÁϳԹÏÍø±¬ÍøÕ¾ College Physical Form

• Students must submit documentation of a physical performed within the last 18 months. Physicals can be documented on a ºÚÁϳԹÏÍø±¬ÍøÕ¾ Physical form OR documentation from your medical record is acceptable.
• NCAA athletes should defer to guidance from Athletics regarding physical exam requirements.
• Students who have limited access to health care resources should reach out to Health Services for guidance.

Request for Provisional Admittance

• To be completed if you have not met immunization requirements by June 20^th

TB Testing Form

• Students answering YES on the Tuberculosis Screening Questionnaire must have their HCP complete this form.

• Log into the .
• Register for the student health portal using your username@bowdoin.edu Enter your password for your ºÚÁϳԹÏÍø±¬ÍøÕ¾ account.

• In the student health portal select the My Forms tab.
• Complete the Medical History and Wellness Form.
• Complete the Immunization History Form using the immunization documentation obtained from your HCP.
• Read and sign the Consent Form & Treatment of a Minor.
  • Must be signed by the student.
  • If you will be under 18 when you arrive on campus, your parent or guardian must sign.
• Complete the Tuberculosis Screening Questionnaire.
• Complete the HS Plan Information Form.

Documents with the following file formats can be uploaded: JPEG, TIF, PNG, PDF, BMP, and GIF. Each file should not exceed 6MB.

Please upload the following documents via the upload button within forms or the Document Upload tab. ​​

• Immunization Documentation
  • Complete immunization records from HCP or school or state registry
  • Upload within the Immunization History Form after entering your immunization dates. ​

• Request for Provisional Admittance
  • Must be completed if you will be receiving missing immunizations over the summer
  • Must be completed if you are an international student who will need vaccines that are not available at home.

• ​Physical Exam Documentation
  • Documentation of a physical OR a completed ºÚÁϳԹÏÍø±¬ÍøÕ¾ Physical Form.
  • A physical performed within the last 18 months is acceptable (unless you are an athlete-see below).
  • Athletes must submit the ºÚÁϳԹÏÍø±¬ÍøÕ¾ Physical Exam Form.
  • Upload within the HS Physical Form and submit. ​
• ​TB Form(s) — only if indicated by screening
  • If TB Screening Form says YES, the TB Testing Form must be printed and completed by your HCP.
  • Upload within the TB Screening Form and submit. ​​
• Insurance Cards
  • Students enrolled in ºÚÁϳԹÏÍø±¬ÍøÕ¾ health insurance will receive cards after arrival
  • If prompted by HS Plan Information Form, take photos of the front and back of card.
  • Upload images within HS Plan Information Form.
  • If insurance changes during the school year, please upload a new card.
